Figuratively, the little green cart came to collect you to take you to the 'lunatic asylum' (1940s), after you'd said something considered very silly/funny. A variation where I lived (still in the 40s) was 'It won't be long, it won't be Lewisham' ('Lewisham' referring to the general hospital then existent in that suburb, as opposed to, perhaps Callan Park (Rozelle psychiatric hospital): Informal conversation in a group of friends, when laughter prevailed, and someone engaged in a particularly funny/silly flight of fancy.
